Important

All Patients under the age of 18 must be accompanied by a parent or guardian.

A prescription from your referring physician

Patient Billing

For your convenience, we accept cash, checks, and credit cards. Payment is due at the time service is rendered unless other arrangements have been made in advance.

Insurance

For a list of insurance companies that we support, click on the Insurance tab above.  Please contact the office to verify acceptance of your plan. Insurance plans are an agreement between the Patient and the insurance company. Patients should be aware of their insurance co-payments and limitations. As a courtesy, we will gladly file insurance claims on your behalf. The Patient’s estimated portion is due at the time of service. Please notify the office of any changes or updates to your insurance.

All new Patients: Please bring your insurance information to your first visit.

We proudly accept the CareCredit credit card to help you finance your prosthetic and orthotic needs. CareCredit can be used to cover high deductibles, co-pays, co-insurance, and to pay for treatments and procedures not covered by insurance.

If you do not have a CareCredit credit card, you can click on the Apply Now button to find out immediately if you are approved with their easy automated system.
